Nephrology patient care is experiencing exciting innovation, including changes in technology, in how we care for patients, and in how patients access care. This session focuses on the need to delay kidney disease progression while also supporting treatment modality decisions. Apps and other technological resources are available to assist patients in managing their care. CVS Kidney Care is used as an example of a patient program that is using innovation to advance education and care for patients with kidney disease.

Complete the learning activity in its entirety and complete the online CNE evaluation. You will be able to print your CNE certificate at any time after you complete the evaluation.
