An error has occurred in the dialysis unit. The patient was hospitalized. The nurse who made the error has not been back to the unit since. While patient safety has become a number one priority, humans make mistakes, and the second victim suffers shame, embarrassment, and disgrace for making an unintentional mistake. How do we support a culture where we examine the processes that led to the problem – rather than blame one person when anyone could have made that mistake?
