Tracy Anderson-Haag PharmD, BCPS, BCTXP

Tracy Anderson-Haag is a Clinical Pharmacy Specialist in Kidney Transplantation at Hennepin Healthcare. She is double board certified in pharmacotherapy and solid organ transplant. Tracy is responsible for the care of inpatient and ambulatory transplant recipients and living donors. Tracy is also the Residency Program Director of the PGY2 Pharmacy Residency Program in Solid Organ Transplant at HCMC. Dr. Anderson-Haag serves as the Engagement Co-Lead for the Advancing Kidney Health through Optimal Medication Management (AKHOMM) initiative which aims to ensure every person with chronic kidney disease receives optimal medication management through team-based care including a pharmacist.
