Goal: To provide an overview of the importance of a patient-centered model of care and the role of the nephrology nurse in this model.

Objectives:

Explain the benefits of involving patients in their own healthcare decisions.
Discuss how nurses can help empower patients into becoming involved in their own self-care.
Describe how successful nurse-patient partnerships can be formed.
Identify barriers that may prevent the establishment of a successful nurse-patient partnership.
