Cristina Perez has been a nephrology nurse practitioner since 2015. Prior to that she had 25 years experience as an RN certified in Rehabilitation and specializing in strokes, head injuries, spinal cord injuries and other neurological conditions. Her transition to nephrology started while working for a private practice group in Riverside, Ca where she learned and grew to be passionate about kidney care. In 2023, Cristina joined the Carelon Health team, part of the Anthem/Elevance Health Plan. Her role at Carelon involves providing advanced primary care for the ESRD population but follows patients through the CKD stage 3, to dialysis, and post-transplant continuum.
