Objectives:

  1. Relate the concept of personal preservation to readiness to discuss the advance care plans.
  2. Summarize a tool used to assess the attitudes of patients with chronic kidney disease in relatoinship to personal preservation.
  3. Design a plan for discussing advance care plans with a patient with chronic kidney disease.
