Jodi Conti BSN, RN

Jodi Conti has worked in nephrology for over 20 years. She has worked as a nurse with both in-center hemodialysis and home therapies, she has also managed a nephrology practice but the bulk of her years were spent as a clinical manager until she was approached with the opportunity to start up the Care Navigation Unit with Fresenius.

This department has been developed to coordinate care for multiple value- based programs including payor program agreements, a Chronic Special Needs plan, and ESCOs.

Last year, during this conference, she spoke about transportation obstacles faced by the ESRD population and the use of the ESCO transportation waiver. Her department continues to brainstorm on interventions that can reduce unnecessary hospitalizations for the ESRD patients that we all care for.
