Chantale Daifi PharmD

Dr. Chantale Daifi completed her Doctor of Pharmacy at Purdue University College of Pharmacy in 2017. She started her career at Henry Ford Hospital in Detroit, Michigan, post-graduate residency as the clinical pharmacy specialist where she worked directly with chronic kidney disease and end stage kidney disease patients on medication management, bone and mineral disease management, and hypertension. Dr. Daifi has a few publications, specifically focusing on end stage kidney disease patients, one of which is establishing the role of a pharmacist in a hemodialysis facility. Dr. Daifi received her teaching certificate in 2017 and precepts pharmacy students in an ambulatory care setting. Aside from presenting at national conferences, Dr. Daifi serves as a co-chair on the workgroup for Advancing Kidney Health through Optimal Medication Management. Dr. Daifi continues to work with CKD and ESKD patients in her daily practice with the goal of expanding pharmacists throughout dialysis centers.
