At the completion of this module the learner will be able to:


  1. Identify the differences between Stages 4 and 5 CKD Post-Transplant patient.

  2. Discuss the prevalence of Stages 4 and 5 CKD Post-Transplant patient.

  3. Identify key assessment areas for the 4 and 5 CKD Post-Transplant patient.

  4. Identify potential problems the CKD Stage 4 or 5 Post-transplant patient can encounter and preventative measures to minimize those problems.

  5. Describe the types and rationale for medications used.
