233 - Hospital U-Turns: Preventing Rehospitalizations

This session focuses on strategies to prevent frequent rehospitalizations in patients with kidney disease. Transition care planning, medication reconciliation, care coordination, and the nurse’s role in daily assessments and post hospitalization are addressed.
