I truly appreciated the research that goes into these journals. In my own personal experience of any transfer of care (primary physician retiring) it was stressful and still there are many gaps in continuity of care. I really feel for adolescents transitioning to adult care. With the many changes that they're going through valuable information gets lost in the transfer to adult care. Thanks for bringing this matter to our attention and the need for more research to preserve and prevent graft rejection.
